Blossom Seeds Care
Building on the Cloud Nine initiative with Fei Yue Community Services, the Foundation has supported Blossom Seeds with a new programme to strengthen community-based palliative care through the development of its first home palliative care service.
The programme leverages Blossom Seeds’ Active Ageing Centre and befriending networks to support seniors across the full continuum—from active ageing to serious illness care at home. Alongside clinical services, it will develop a network of trained volunteers, peer companions and caregivers to support patients and families. The goal is to enable more seniors to live and receive care in the community, including through the final stages of life.
