Cloud Nine
Cloud Nine explores how Active Ageing Centres (AACs) can serve as community anchors that support seniors from ageing well to the end of life.
As AACs expand across Singapore, the initiative seeks to integrate palliative care principles within everyday community programmes, enabling seniors with chronic or life-limiting conditions to remain connected to familiar spaces and support networks. The pilot will work across multiple AACs to develop a “through-train” model of care, with structured transitions to specialist palliative providers when needed. It also explores how community spaces can be designed to better support individuals living with serious illness.
