Post Diagnostic Support Plus
This pilot strengthens support for persons living with dementia by integrating palliative care earlier within existing dementia services.
It focuses on individuals discharged from Dementia Day Care Centres who often face a gap in care once they can no longer participate in centre-based programmes. The initiative introduces advance care planning, transitional palliative support and closer coordination with primary care to better support both patients and caregivers as needs evolve. Together, these elements aim to create a more continuous pathway of care for people living with dementia across the later stages of the disease.
