Each year, about 10,000 Singaporeans suffer a stroke. It is the nation’s third leading cause of disease burden, with one in four people expected to experience a stroke in their lifetime.
Yet recovery is often limited—only one in three survivors achieves partial recovery, and progress is greatest within the first 3–6 months. Despite recommendations for two therapy sessions a week, only about 30% – 40% of survivors take up community rehab, hindered by poor coordination, lack of awareness, and transport barriers.
The Stroke Rehab Ecosystem is a four-year, S$6.59 million initiative led by the Lien Foundation, Singapore General Hospital, and Tan Tock Seng Hospital, in partnership with AWWA, NTUC Health, Stroke Support Station (S3), St Luke’s ElderCare, and SPD. It aims to cut post-discharge wait times from months to just two weeks, while also strengthening mental health and wellness support.
More than 2,000 survivors will benefit through direct referral pathways, dedicated care coordinators, and better-trained community therapists. A centrepiece is Singapore’s first adaptive gym, operated by S3, helping survivors sustain recovery and regain quality of life. Together, hospitals and community partners are building a more holistic and sustainable model of post-stroke care.
