As people live longer with dementia, their needs can change considerably over the course of the condition. PDS+ builds on the earlier Post Diagnostic Support programme by extending support further along the dementia journey, particularly as care needs become more complex and centre-based services may no longer be suitable.
Delivered by Dementia Singapore, PDS+ integrates a precision person-centred palliative approach earlier in the dementia trajectory. It introduces Advance Care Planning while clients are still attending dementia day care and provides transitional and home-based support as care shifts to the home. A dedicated multidisciplinary palliative care team works with clients, caregivers and healthcare providers to anticipate changing needs and provide continuity of care.
The programme also builds capabilities beyond direct service delivery, through staff training, practical palliative care resources and tools that can be shared with the wider dementia care sector.
